Acknowledgment of Responsibility

By submitting this form, I acknowledge that I did not properly fulfill my job responsibilities. I understand that simply being present at an aquatic facility does not constitute “working.” Failure to clock in and out using SecureTime in the GoAquatix App is a failure to perform my job.

I recognize that I am required to document the following four items:

1. Hours Worked
2. Swim Tests
3. Hourly and Daily Opening/Closing Duties
4. Required Reports

Failure to complete any of these responsibilities is considered a failure to do my job.

I also understand that I am not automatically entitled to the pay I am requesting. Instead, I am asking the company to manually review and research my request, which may take up to four (4) weeks to process. This request will be subject to minimum wage rules, if approved.

I confirm that I have read and understand the Company’s Payroll Packet, and I agree to all related terms and conditions.
